Successful Outcomes After Release
Services provided: to residents (with a court record other than minor traffic violations) currently living in the Winston Salem,N.C./Forsyth County area.
We provide assistance with locating and retaining permanent housing. Participants must have income sufficient to afford to pay market rent for the permanent housing unit they select. We do not own or manage housing units. We only assist the participant in the search for permanent housing that may be listed on the open market. We do not assist with the following housing: rooming house, transitional housing, hotel, or homeless shelters.
Other services include the following: payments towards past due rent, rent deposits, utility bills or deposits. Bus transportation to work, shoes for work and food items. The maximum amount of financial assistance is $300.00. It can be used for more than one category of needs listed above.
Required Documents:
Rental Lease with your name and the landlord listed with mailing address on it.
a. (If the lease is pending, based upon payment of the rental deposit, you can submit a statement to that fact from your future landlord including name and address.
Photo ID of yourself. Do not send Social Security Information.
Statement from the court, landlord, or utility company stating how much you owe.
This must include:
account number and company address.
4. Criminal Background report (non-certified)
copy, not more than 3 pages) from the
(police department or courthouse).
Please note: Minor traffic violations do
not qualify for this program.
